Joint Vibrational Analyzer

In this age of advanced technology, a clinician must rely on more than subjective findings (e.g. palpation and auscultation) when evaluating the temporomandibular (jaw joint). We need a way to objectively assess our patient's joint health and document both the prior to the treatment conditions and the response to the treatments we provide using mechanical devices.

One of the personal computer based tools available to evaluate temporomandibular joints is JVA,* Joint Vibration Analysis.

The existence of this type of evaluation of the jaw joint in function is critical for objective (technologically derived), diagnostically driven treatment Joint Vibration Analysis (JVA) is based on simple principles of motion and friction: When smooth surfaces rub together, little friction is created... and thus little vibration.

However, if surfaces become rough, then friction causes vibrations when these surfaces articulate (Meet Together).

The T.M.J (Jaw Joint) is a ginglymo-arthroidial joint with surfaces that glide together in function which means that the 2 parts of the joint function as a single unit. The smooth, well-lubricated surfaces in a healthy joint have a bio mechanical relationship that produces very little friction and almost no vibration.

Surface changes, such as those caused by subtle degenerations, any perforations or mechanical displacements generally produce friction and some vibration. Different disorders produce different vibration patterns or “Signs” or Signatures. Joint Vibration Analysis Analysis helps the clinician identify these conditions from the vibration patterns and helps distinguish a primary TMJ (Jaw Joint) dysfunction or irregularity from other painful conditions.

When we hear sounds, we distinguish one sound (vibration) from another by their differing amplitudes (loudness), duration (long vs. short sounds) and pitch, harmonics, etc. (sound qualities). Joint Vibration Analyzer does the same thing, but more accurately, without any subjectivity, with honest reproducibility and providing a long term record that is available for valid comparisons in the future.


Are vibration and sound the same?

Well, yes and no. All audible sounds come from vibrations, but not all vibrations produce an audible sound. In fact, our ears are simply incapable of hearing joint vibrations at the low frequencies that some important joint pathologies produce. We may also be confused by the combined sounds of two conditions present in the same joint or the side it's on. This is probably why research studies show that auscultation has about the same accuracy as random chance.1-4 Furthermore, ears (and microphones, incidentally) pick up room sound and other artifacts, where JVA picks up only vibrations from the joint itself.

Traditionally, we have used palpation and auscultation to detect TM joint "sounds". Auscultation is dependent upon the hearing ability of the examiner and is limited to unilateral testing with no long term record. The interpretation of these "sounds" has been shown to be very difficult -- "What type of sound was it?" -- "Did it occur upon every opening?" "Which side did the sound occur on?" Palpation is a skill with a steep learning curve that requires great tactile sensitivity and suffers from a low specificity. Even though it is usually done bilaterally, it can be very difficult to distinguish which side is causing the joint sound.

Joint Vibrational Analyzer is a great screening test since it has such a high specificity. It is also the ideal, low cost way to monitor joint function during the course of treatment. While it does not eliminate the need for expensive imaging, it allows the practitioner to make a more informed decision whether the cost of imaging is justified.

The Jaw Tracker is an instrument that measures and records the movements of the jaw. Hence any discrepancy in the jaw movements can be analyzed using this. Hence it can be used to detect any irregularity of the jaw joint, called T.M.J disorders. Disorders of the Jaw can lead to several body pain chronically as well.

One which we believe in is the one promoted by Dr. Sanjay Arora , of Zental - Center For Relief from Body Pains and that states,” There are several nerves and blood vessels in an inch of space under the jaw joint”. The nerve that controls the position of head, (No. 11 or accessory nerve), lies fairly close to the joint. Irritation of this nerve re-positions the head on the shoulder.

The jaw normally moves without any wobble on the teeth. Whenever there is a grinding of teeth more on one side than the other, then the jaw settles one side first, the same mimicking in the jaw joint.

Once he determines what’s wrong, he creates a diagnosis and then formulates a treatment plan.

If you are primarily a dental patient, he may decide to take you to the stage2 of the treatment. If you are not primarily a dental patient, he may decide to take you through the phase 1, where he will construct a device to be worn in the night time essentially. In the night time the force and scale of movement of jaws is very high. This gives rest to the structures around and under the joint.

Once you are relieved from Phase 1, Phase 2- which is a long term phase is initiated due to temporariness and long term harmful effects of Phase 1.

It is expected, that the entire therapy should start giving relief in 15-30 days or as some report earlier. While the medical field is very vast and one cannot master all that is possible, we do experience a very high number of patients responding to these therapies.
